How to be
prepared
It is helpful to
come to an appointment prepared, as best you can, with the information
needed to properly allow us to evaluate the legal issues that concern you.
The following checklist of items would be helpful in evaluating your case.
This is not a prerequisite for your making or coming to an appointment.
Forms are available for you to bring to your appointment if you so desire.
All information that you bring to the appointment will be strictly
confidential and is covered by the attorney-client privilege. However, do
not send this information prior to your appointment. Until the
attorney-client relationship is established, there may not be the privilege
of confidentiality. The following are helpful tips for our clients:

provide his or her death certificate.

spouse, child, grandchild. If the decedent had an address book, please

(7) Conservatorship
(1) Information and background sheet
(2) Copy of any Trust Agreement, Last Will and Testament, Power of Attorney,
and health care directives of the proposed conservatee, if available.
(3) If any spouse, child, grandchild, parent, or sibling has died, please
provide the death certificate. (Only if readily available; if not, then, if
possible, provide the date and location of death.)
(4) Deeds and property tax statement to any property owned by decedent
(5) Automobile, motor home, boat, trailer registration or certificate of
title
(6) Current bank account statements
(7) Stock certificates and current broker account and mutual fund
statements
(8) Life insurance policies

conservatee
(11) Last two years' income tax returns
(12) Addresses of any living spouse, child, grandchild, parent, and sibling
of the proposed conservatee. If the proposed conservatee has an address
book, please provide it at the first appointment.
(13) Name, address, and any reports of any treating physician of the
proposed conservatee
(14) If problems with elder abuse any police reports relating thereto

(2) Birth certificate of proposed ward
(3) Description of any asset of value owed by the ward or to which the ward
is entitled
(4) Address of any living parent , sibling, or grandparent of the proposed
